How This Protocol Works

Simple Explanation

IBS involves a disrupted connection between your brain and gut. Your intestines become hypersensitive, overreacting to normal signals with pain, bloating, and altered bowel habits (diarrhea, constipation, or both). The gut microbiome is often imbalanced, and the enteric nervous system sends too many pain signals.

Peppermint oil (enteric-coated capsules) relaxes the smooth muscle of your intestines by blocking calcium channels. This stops painful spasms and cramping. Studies show significant improvement in abdominal pain—it's one of the best-studied IBS treatments.
Probiotics restore healthy gut bacteria balance. They strengthen the intestinal lining, reduce inflammation, and improve gut-brain communication. Multi-strain formulas or specific strains like B. infantis 35624 work best.
Psyllium fiber is a soluble fiber that helps normalize bowel movements in both IBS-D and IBS-C. It absorbs water to bulk up loose stools and softens hard stools.
Iberogast is a German herbal combination that has been used for decades. It calms intestinal spasms, reduces inflammation, and addresses both stomach and intestinal symptoms.

Expected timeline: Peppermint provides relief within 1-2 hours of dosing. Probiotic benefits develop over 4-8 weeks. Fiber effects begin within days but require consistent use.

Clinical Perspective

IBS pathophysiology involves visceral hypersensitivity, altered gut motility, dysbiosis, increased intestinal permeability, and dysregulation of the gut-brain axis. This protocol targets multiple pathogenic mechanisms.

Peppermint oil (A-grade): Menthol antagonizes L-type calcium channels in intestinal smooth muscle. Produces direct antispasmodic effect independent of anticholinergic mechanisms. Reduces colonic motility index. 12 RCTs with 835 participants show NNT of 4 for global symptom improvement (PMID: 24100754). Enteric coating essential to prevent heartburn.
Probiotics (B-grade): Competitive exclusion of pathogenic bacteria. Production of SCFAs supporting colonocyte health. Enhancement of tight junction proteins reducing permeability. 16 studies show moderate improvement in global IBS symptoms (PMID: 28537379). B. infantis 35624 reduces inflammatory markers.
Psyllium (B-grade): Soluble fiber increases stool bulk via water absorption. Fermentation produces butyrate supporting intestinal health. 6 studies show improvement in stool consistency and frequency (PMID: 19713235). Use soluble fiber; insoluble fiber may worsen symptoms.
Iberogast/STW 5 (B-grade): Nine-herb combination includes iberis amara, peppermint, chamomile. Modulates serotonin receptors affecting GI motility. Reduces visceral afferent sensitivity (PMID: 17206180).

Biomarker targets: Stool consistency (Bristol scale), abdominal pain VAS, IBS-SSS score, fecal calprotectin.

Protocol notes: IBS subtyping guides therapy—probiotics for all subtypes; psyllium adjust based on stool form. Low-FODMAP diet often synergistic.
